Coordinated Family Support (CFS)

Individualized support for adults living with family—coordinating services and resources, developing skills, accessing the community, and addressing barriers to their goals and continued living at home. Support is flexible and respects your culture and individual preferences.

Funding & service arrangement

Self-Determination Program (SDP)

A funding arrangement that gives participants greater choice and control over services supporting their Individual Program Plan (IPP) goals. Independence Plus provides agreed-upon support within the approved funding arrangement.

Planning your support

Where do you provide support?

Sessions can take place at home, on campus, or in the community, depending on your goals and authorized support. Email or call with your city and preferred setting so our team can confirm current service availability. Location, staffing, travel, and the applicable funding arrangement need to be discussed before services are agreed.

What might a session look like?

You and your instructor work on a goal that matters to you. That might mean planning and preparing a meal, practicing a bus route, organizing a weekly budget, preparing questions for a college office, or taking part in a community activity. We adapt teaching and practice to how you learn. The activities and schedule depend on your support plan and authorization.

What determines when services can start?

We first discuss your needs and service fit, complete the assessment process, confirm the necessary authorization or funding arrangements, and arrange an available instructor and schedule. These steps determine the start date; an inquiry or referral does not guarantee immediate placement.
